The oral cavity harbors several Streptococcus mutans genotypes, which could present distinct virulence properties. However, little is known about the diversity and virulence traits of S. mutans genotypes isolated in vivo under controlled conditions of high cariogenic challenge. This study evaluated the genotypic diversity of S. mutans isolated from dental biofilms formed in vivo under sucrose exposure, as well as their acidogenicity and aciduricity. To form biofilms, subjects rinsed their mouths with distilled water or sucrose solution 8 times/day for 3 days. S. mutans collected from saliva and biofilms were genotyped by arbitrarily-primed PCR. Genotypes identified in the biofilms were evaluated regarding their ability to lower the suspension pH through glycolysis and their acid susceptibility and F-ATPase activity. Most subjects harbored only one genotype in saliva, which was detected in almost all biofilm samples at high proportions. Genotypes isolated only in the presence of sucrose had higher acidogenicity than those isolated only in the presence of water. Genotypes from biofilms formed with sucrose were more aciduric after 30 and 60 min of incubation at pH 2.8 and 5.0, respectively. The present results suggest that biofilms formed under high cariogenic conditions may harbor more aciduric and acidogenic S. mutans genotypes.  相似文献

The timing of surgery for velopharyngeal dysfunction has been based on assumptions about the relation between age, speech development, and velopharyngeal dysfunction. Cleft palate teams often counsel parents to have an intervention for velopharyngeal dysfunction performed earlier rather than later, believing that earlier interventions result in more rapid or better normalization of speech. The objective of this retrospective chart review study is to determine whether the age at surgical intervention for velopharyngeal dysfunction has an effect on the subsequent length of speech therapy. Of 174 patients included in the study database, 36 had velopharyngeal dysfunction for which further velopharyngeal management was required. Of the 36 patients who received surgical velopharyngeal dysfunction management, 27 had verifiable speech therapy records. These 27 patients represent the study population. The outcome measure was the total length of subsequent speech therapy until speech normalization. The data suggest that there is no relation between the age at velopharyngeal dysfunction surgical management and the amount of speech therapy needed to achieve normalization of the speech impairments secondary to velopharyngeal dysfunction after that management. In conclusion, 1) the age at surgical velopharyngeal dysfunction management (pharyngeal flap or sphincter pharyngoplasty) does not have an effect on subsequent normalization of speech as measured by the duration of speech therapy necessary to achieve normalization of the speech impairments secondary to velopharyngeal dysfunction after that management, and 2) the age at surgical velopharyngeal dysfunction management does not affect the likelihood of subsequent surgical velopharyngeal dysfunction management procedures.  相似文献

Objectives

The purpose of this study was to evaluate the cantilever-bending test as a bond-strength measurement method for enamel/composite adhesion.

Materials and methods

The buccal surface of bovine incisors were flattened and subsequently placed with composite Z250 (3M ESPE) incrementally after either self-etching adhesive (SE) or etch-and-rinse adhesive (SB) was applied. Then, they were cut with a low-speed cutter to obtain stick-shape samples. The samples were divided into four groups (n = 30 each) according to the adhesive and test method: SET, SE-B, SB-T and SB-B, where T denotes the microtensile test and B denotes the cantilever-bending test. The failure mode of each sample was examined under the microscope. Those samples with the fracture surface lying entirely or partially in the adhesive layer were considered to have provided successful measurements. The results from the two test methods were compared.

Results and discussion

The cantilever-bending method produced a comparable success rate of bond strength measurement to that of the microtensile method. The bond strengths of SE and SB from the cantilever-bending test were 58% and 40% higher, respectively, than those measured with the microtensile method. The adhesive SE exhibited almost the same bond strength as SB, irrespective of the test method.

Conclusions

Cantilever bending can be used as an alternative method for bond strength measurement. The ratio of bending to tensile strength is around 1.5.  相似文献

Although supervised night-guard bleaching has proved successful for whitening teeth, both mineral loss and tooth sensitivity are still common problems. The present study introduces a novel remineralizer as well as describes the utilization of a possibly low sensitivity supervised night-guard vital tooth bleaching technique. A 22% carbamide peroxide gel was mixed with the casein phosphopeptide-amorphous calcium phosphate-containing MI Paste. The peroxide/MI Paste mixture was prepared with 1 ml of each material until obtaining a homogeneous paste. Five patients were instructed to follow a home bleaching protocol for 3 weeks. The outcome was assessed visually with a Vitapan scale. All subjects presented reduction of at least two Vitapan scale units after bleaching and no sensitivity was reported. The concomitant use of MI Paste and peroxide might not affect the gel effectiveness and still reduce hypersensitivity levels.  相似文献

This study quantified microleakage in restorations made with three packable resin composites-Solitaire, SureFil and P60; one hybrid resin composite-Z250 and an amalgam-Dispersalloy, with or without the thermocycling process. Sixty sound, freshly extracted human molars were sectioned mesiodistally, creating buccal and lingual blocks. One hundred blocks with the flattest surface were selected. Cylindrical cavities with a diameter of 1.85 +/- 0.05 mm and a depth of 1.5 mm were prepared with a special diamond bur. The blocks were randomly assigned to 10 test groups (n = 10): five restorative materials and two thermal stress groups (thermocycled groups at 3,000 cycles at 5 degrees C and 55 degrees C with a dwell time of one minute at each temperature, or non-thermocycled). After the thermocycling test, the samples were immersed in 2% methylene blue for 12 hours. The samples were ground and the powder prepared for analysis in an absorbance spectrophotometer. All the results were statistically analyzed by the Kruskal-Wallis test and the Mann Whitney test. For the non-thermocycled groups, the means (microg/ml) of microleakage were: Amalgam-4.279 (a); Solitaire-4.148 (ab); Z250-3.418 (abc); P60-3.184 (bc); SureFil-2.890 (c). For the thermocycled groups, the means were: Amalgam-7.572 (a); Solitaire-5.471 (a); Z250-4.330 (ab); P60-3.418 (bc) and SureFil-2.779 (c). Thermocycling analysis showed no significant differences between the thermocycled and non-thermocycled groups for each material tested. It was concluded that no test material prevented microleakage. Only SureFil and P60 showed leakage means significantly lower than amalgam, with SureFil showing lower leakage than Solitaire. P60 only showed lower leakage than Solitaire in the thermocycled groups and Z250 showed results similar to the others materials.  相似文献

Objective

This paper studied in vitro the effect of the C-factor on interfacial debonding during curing of composite restorations using the acoustic emission (AE) technique. Finite element (FE) analyzes were also carried out to evaluate the interfacial stresses caused by shrinkage of the composite resin in restorations with different C-factors.

Materials and methods

Twenty extracted third molars were divided into 4 groups of 5. They were cut to form Class-I (Groups 1 and 2) and Class-II (Groups 3 and 4) cavities with different C-factors. The average C-factors of the four groups were 3.37, 2.88, 2.00, and 1.79, respectively. The cavities were then applied with an adhesive and restored with a composite, which was cured by a halogen light for 40 s. A 2-channel AE system was used to monitor the interfacial debonding, caused by shrinkage stress, between the tooth and restoration through an AE sensor attached to the surface of the specimen. Recording of the AE started at the same time as curing of the composite and lasted 10 min. Simplified FE models were used to evaluate the interfacial stresses in restorations with different C-factors, with a thermal load (temperature decrease) being applied to the composite resin to simulate its shrinkage.

Results

The mean and standard deviation of the total number of AE events for the four groups were 29.6 ± 15.7, 10.0 ± 5.8, 2.6 ± 1.5, and 2.2 ± 1.3, i.e. the number of AE events increased with an increase in the C-factor. The FE results also showed that, the higher the C-factor of the restoration, the higher the interfacial tensile stress between the tooth and restoration.

Significance

From the results of the AE tests and FE simulations, it can be concluded that, the higher the C-factor, the higher the shrinkage stress and the more likely is interfacial debonding.  相似文献

There is little information on the oral health of children undergoing renal transplantation during the early transplant period. Methods. Twenty‐four children undergoing renal transplantation aged 4–13·2 years and their matched controls were recruited. The dmfs, dmft, DMFS and DMFT, plaque, gingivitis and gingival enlargement scores were recorded. The oral microflora was sampled and cultured for S. mutans, Lactobacllus species and Candida species. Results. There was a significantly lower mean dmfs (0·3 ± 0·9; P = 0·03), dmft (0·3 ± 0·9; P = 0·03), DMFS (2·3 ± 5·3; P = 0·01) and DMFT (1·5 ± 2·6; P = 0·02), respectively, in the transplant group. There was a significantly greater mean plaque score (14·7 ± 11) for the permanent dentition, at baseline only, compared with 90 days post‐transplantation (9·4 ± 10·4; P = 0·02). There was a significantly greater gingival enlargement score (1·8 ± 1·4; P = 0·04) 90 days post‐transplantation compared with baseline. The S. mutans and Lactobacillus counts were significantly lower both at baseline (P = 0·0001 and P = 0·004) and 90 days post‐transplantation (P = 0·02; and P = 0·05), respectively, compared with the controls. Conclusions. The transplant children had less active dental disease than the controls although gingival enlargement needs careful monitoring.  相似文献

Internal root resorption is an uncommon lesion following a dental injury. The use of mineral trioxide aggregate (MTA) is a conservative approach to repair lesions with periodontal communication. This case report presents a long-term follow-up of a nonsurgical endodontic management using MTA for perforative defect of internal root resorption. During the endodontic treatment, the granulation tissue was removed and the root canal prepared. Calcium hydroxide was placed as a temporary dressing for 30 days. After this period, the root canal space and the perforation defect were filled with MTA. The clinical findings and periapical radiographs indicated success of treatment until 2 years of follow-up. However, the radiograph after 8 years showed an extensive radiolucent area in the middle third of the root with separation of the apical and coronal root segments. These findings were observed more accurately by using cone-beam computerized tomography.  相似文献
